The Hospital at Home service is an innovative and pioneering service based at the Abingdon Community Hospital in Oxfordshire, covering a large catchment area across Central, West and South Oxfordshire. The service works collaboratively with Services from the OUH, to provide a holistic approach to assessment, and provide hospital level treatment for patients in their own homes, thus avoiding admission or expediating discharge from secondary care providers.

We are seeking autonomous enthusiastic, and patient focused Practitioners to join our multi disciplinary team, as Non Medical Prescribers, to further develop and allow the H@H Team to offer hospital level care within patients own homes. The service runs 08.00 - 22.00 7 days a week, 52 weeks of the year. Flexibility within these hours will be required.

Main duties of the job

Working with a high degree of autonomy, the post holder will:

Manage undifferentiated and undiagnosed conditions by using, developing and consolidating advanced clinical skills within their own sphere of competence.

Support remote management of patients conditions, who otherwise would need to be transferred to an acute care setting to receive assessment and treatment.

Provide holistic patient care, in the patient's residence, from assessment of presenting condition through to diagnosis, hospital level treatment and then discharge, including the prescribing and administration of medication, thus providing a alternative to admission into acute care services.

Work collaboratively within the wider multidisciplinary team and with external providers, to ensure the right care is provided, by the right person, at the right time and in the right place.

Job responsibilities

Oxford Health Foundation Trust is part of the Buckinghamshire, Oxfordshire and Berkshire integrated care system (BOB), and Oxon Urgent Community Response service was a recognised national accelerator site in the Ageing Well Programme in development of new NHS Urgent Community Response services.

You will help play a key role in continuing to help people manage their long-term conditions, making sure they receive the right kind of support to help them remain independent and live longer and linking in with our healthcare and charity based colleagues in the wider community.

You will be working alongside our Community Nursing and Therapy Teams, South Central Ambulance Service, General Practice and Oxford University Hospital colleagues with the opportunity to work across many facets of Community services.

Qualifications
  • Registered General Nurse or Paramedic
  • Post Registration Qualification in advanced clinical assessment skills e.g. Advanced history taking and examination or Frailty (comprehensive geriatric assessment)
  • V300 (Non Medical Prescribing) Qualification.
  • Mentorship qualification or teaching or equivalent
  • Qualification in Related field.
Experience
  • Competent in using Evidence Based Practice
  • Experience in advanced assessment of patients
  • Autonomous clinical practice
  • Consolidated Prescribing experience
  • Community experience/ home visiting
  • Flexible, adaptable and willing to change with the evolving service
