The Transitional Care Pharmacist plays a vital role in the multidisciplinary team at Hendrick Medical Center

• Collecting accurate medication histories from patients and reliable sources upon admission.
• Ensuring hospital providers have complete medication information for optimal treatment.
• Monitoring patients throughout their hospital stay to support discharge planning.
• Coordinating with outpatient pharmacy services to fill discharge medications.
• Assisting patients with medication access issues, including financial barriers.
• Collaborating with providers to confirm all necessary discharge prescriptions.
• Providing patient education on medication regimens before discharge.
• Opportunities to work across various hospital departments and specialty outpatient clinics.

• Experience: No prior experience required
• Licenses/Certifications: Active license as a Registered Pharmacist in the State of Texas
