Practice Nurse

An exciting development opportunity has arisen for a registered nurse to join the practice team and develop the knowledge and skills required as a General Practice Nurse (GPN).

General Practice Nurses (GPNs) plan and provide care, treatment and health education to patients of all ages.

In this role you will be administering childhood Immunisations, and deliver other immunisation programmes, well woman clinics etc

They are key members of a multi-professional team that includes doctors, pharmacists, therapists and a range of other professionals delivering person-centred care.

We are a friendly practice, staff welfare is very important to us.

We are a training practice. We have 5 GP partners, 1 salaried doctor

GP registrars, Nurse prescriber / manager, practice nurse and a HCA

An amazing admin and reception team.

You will ensure that all nursing care provided to the practice population is evidence based, relevant, of a high standard and effective.

The practice nurse will undertake a range of nursing assessments, including chronic disease management and provide appropriate treatments. You will be supported by your nursing team, practice manager and Lead GP. You will be based at Padarn surgery.

The ability to speak Welsh is desirable for this post;
English and/or Welsh speakers are equally welcome to apply.

• * 1st Level Registered Nurse.
• * IT literate.
• * Experience of leading on management of patient chronic disease registers.
• * Experience of Primary Care Health Working.
• * Relevant degree and /or professional diplomas - or working towards.
• * NMC Mentor and assessor.
