Position: LJR238717 - Community Nurse

Overview

A Band 5 Community Nurse position has arisen within the Portlethen District Nursing Team. This role involves delivering high‑quality, person‑centred care to patients in their own homes and local communities, helping them manage long‑term conditions, recover from illness and live independently.

Key Responsibilities
  • Deliver a range of nursing interventions, including wound care, chronic disease management, medication administration and palliative care.
  • Develop, implement and review care plans in partnership with the Senior Community Nurse.
  • Collaborate with a multidisciplinary team, including doctors and care managers, to ensure holistic, patient‑focused care.
  • Perform clinical tasks such as injections, dressings and monitoring of vital signs.
  • Manage your own caseload independently and maintain accurate clinical records.
Qualifications and Experience
  • Registered nurse with current NMC registration.
  • Strong clinical skills, particularly in wound care, medication administration and palliative care.
  • Excellent communication skills and ability to build relationships with patients, families and team members.
  • Ability to work independently and manage your own caseload.
  • Full driving licence and own car.
Working Hours and Pay

From 1 April 2026 the Agenda for Change full‑time working week will be reduced from 37 to 36 hours per week. Part‑time hours will be reduced proportionally, with a corresponding increase in hourly rate so pay is not affected.
