Position: MDS Coordinator LPN - $5,000 sign-on

Overview

Grays Harbor Health and Rehab in Aberdeen is seeking an experienced MDS Coordinator. This position is on-site.

Salary: $39.00 - $44.62 per hour.

Employment type:

Full-time or Part-time. A $5,000 sign-on for full-time available.

Previous MDS experience required. Use your nursing skills to serve others in our facility. Placement within the wage range is determined by years of experience, qualifications, and demonstrated ability to perform the essential functions.

Responsibilities

The Resident Assessment Coordinator will be responsible for the facility's Medicare and Managed Care residents as directed by the Resident Assessment Director. This includes oversight of all services provided during inpatient stays from admission to discharge. The role is responsible for the accurate and timely completion of all OBRA-required assessments. This position is a key financial and clinical member of the management team.

Qualifications
  • Must have a current, active license to practice as a Licensed Practical Nurse (LPN) in the state of practice.
  • Strong interpersonal communication skills.
  • Familiarity with the RAI process and Case Management.
  • Ability to read, write, and speak English.
  • Must meet all local health regulations and pass a post-employment physical exam if required. This includes drug screening, criminal background investigation, and reference inquiry.
