Overview
The Hospital at Home service within NHS Grampian delivers consultant-led, hospital-level multidisciplinary care to patients within their own homes, across Aberdeen City and Aberdeenshire. Patients can be admitted to Hospital at Home as an alternative to hospital admission, or to support early discharge from acute services.
Responsibilities- As a Band 6 physiotherapist within the Hospital at Home team, contribute to the comprehensive geriatric assessment of frail adults and provide subsequent appropriate treatment.
- Provide respiratory and functional assessments and treatments to acutely unwell adults within patients’ homes.
- Work closely with other community physiotherapy teams within Aberdeen City Health and Social Care Partnership; may be required to work within different areas of community physiotherapy including the Community Adult Assessment and Rehabilitation Service and Discharge to Assess.
- Maintain a varied caseload and develop both acute and community working skills; contribute to service developments.
- Be supported by band 7 physiotherapists and have opportunities to supervise and appraise assistant practitioners and students.
- Learn from a large multidisciplinary team.
- Good frailty, acute medical and respiratory skills and knowledge; must meet the person specification for consideration.
- Valid UK driving licence and access to a car for work purposes.
- This post operates within Aberdeen City Low Emission Zone; you will be responsible for all costs associated with entering.
The working hours for this post will include weekend working as part of a rota system.
Additional information for candidates- As from 1/4/26, the Agenda for Change full-time working week will be reducing from 37 to 36 hours per week. Part-time hours will be reduced pro-rata, with a corresponding increase in the hourly rate so that pay will not be affected.
